Application forms may be requested from the Registrar's office or downloaded from this web site. The completed form should be returned with other required documents before January 31st of the year in which admission is sought.

Applicants seeking entry into the Master of Divinity program must also submit a completed ecclesiastical recommendation form and an academic recommendation form, to be completed by a former instructor, along with your application.  Click on the forms below to print out a copy:

Auditing

Applicants who wish to audit courses on a full-or part-time basis, should click on the appropriate form below to print out a copy:
